Take the following steps or watch the video below to complete a Guest Parking Permit Request Form:

  1. MyCI login box
  2. Click the 鈥淕uest Parking Permit Request Form鈥 on the myCI Services page.

  4. On the Power Form Signer Information page, enter your name and email address under 鈥淩equestor,鈥 and enter the name and email address of the department or division head responsible for approving your request under 鈥淒epartment/Division Head.鈥Powerform signer information page

  5. Note: If you aren鈥檛 sure who the authorized approver is for your request, you can contact Transportation and Parking Services (TPS) at parking@csuci.edu. TPS maintains a list of authorized approvers.


  6. Once you鈥檝e entered the required names and email addresses, click the 鈥淏egin Signing鈥 button in the bottom right corner.begin signing button

  7. Next click the 鈥淐ontinue鈥 and 鈥淪tart鈥 buttons to begin filling out the form.

    Note: If this is your first time to use DocuSign to complete a form here at 华体会, you will be required to first review and accept the Electronic Record and Signature Disclosure at the top of the screen before you can continue.


  9. Fill out the form fields with the information requested. Any fields outlined in red are required fields and must be completed before sending on the form.

  11. Parts of the form allow you to attach documents. Just click on the paperclip icon, click on 鈥渦pload a file,鈥 then find the file you want to attach and click 鈥渙pen.鈥 Press done to complete attaching the document. The document you attached will then appear below the form.

  13. Once you鈥檝e completed all the required fields in the form, click the 鈥淔inish鈥 button that is found either at the bottom of the page or the upper right corner.

  15. After your approver has signed the form, it will then be routed to TPS for review.

  16. You will be notified via email once your request has been approved by TPS.
